Membership Terms and Conditions

Effective Date: 1/1/2025

By enrolling in the monthly membership program, the Client agrees to the terms listed below.

1. Membership Policy

This membership requires a minimum commitment of three (3) consecutive months. After the 3-month minimum term has been fulfilled, the membership will continue on a month-to-month basis unless cancelled by the Client in accordance with this Agreement.

2. Payment & Billing

The Client authorizes the Provider to automatically charge the card on file for the recurring monthly membership fee. Payments are processed on the same day each month based on the original enrollment date.

3. No Refund Policy

All membership payments are final and non-refundable, including payments for unused services, early cancellation, or schedule conflicts.

4. Cancellation Policy

The Client may cancel at any time by submitting at least 30 days prior to the next billing date.

5. Early Termination Fee

If the Client cancels before completing the 3-month minimum term, a flat Early Termination Fee of $150 will be charged. This fee is due immediately upon cancellation and is non-refundable.

6. Missed Payments / Declined Card

If a payment fails, the Client must update payment information within 7 days to avoid interruption of services. Provider reserves the right to pause or terminate membership benefits until balance is paid in full.

7. No Transfer or Sharing

Membership benefits cannot be shared, transferred, gifted, or used by anyone other than the Client listed on this Agreement.

8. Liability & Acknowledgment

Client understands that all services may involve risks and agrees to sign required consent forms prior to services. Provider is not responsible for adverse reactions or outcomes outside standard care.

9. Agreement

By agreeing, the Client acknowledges they have read, understood, and agree to the terms of this Membership Agreement and authorize monthly recurring billing.
